• Technology Support Request
  Staff - use this form to request assistance with any office computing problems
  Students - use this form to request assistance with ResNet and BuckID issues
• Facilities Service Request
  Staff - use this form to request assistance with maintenance, housekeeping or facilities in a Student Life office building
  Students - use this form to request assistance with maintenance, housekeeping or facilities in your residence hall room
  Your BuckID number will be required to submit this page.
• Computer Account Request
  Staff - use this form to add, modify or delete a computer, email, Web-based Timekeeping and/or any other account for a Student Life employee
  Your Student Life login (the username and password you use to log onto your computer, for example, username is "studentaffairs\j12345") will be required to access this page.
• Application Work Request
  Staff - use this form to request any work from Student Life IT Application Development; this work includes any new websites or web applications or enhancements to existing websites or web applications.
• Hardware/Software Purchase Request
  Staff - use this form to purchase new and/or upgraded hardware (workstation, laptop, printer, etc.) and upgraded software (Microsoft Word, Excel, Adobe Acrobat, PhotoShop, etc.)

• Landline Phone Service Request
  Staff - use this form to have office phone service added, changed, moved, transferred or removed for one of your employees
• Wireless Phone Service Request
  Staff - use this form to have wireless phone service added or changed for one of your employees
• Facilities Project Request
  Staff - use this form to request a change in the function of a room; to construct or relocate doors or walls; move existing furniture; request furniture, equipment and decorating; or request new space
