This request form can be used to initiate or change wireless phone service.
To request landline phone service or changes use the Landline Phone Request Form
For damages or repairs please use the Technology Support Request Form or contact the support center at 292-5700.
Approval will be required by your department.
Please review the Student Life Cell Phone Procedures.

Texting (In Addition to Phone Charges): $20/month - AT&T, $15/month - Nextel, $15/month - Verizon, $10/month - if Taxable Allowance

Business Only Use (Not Valid for Data Plans)
Taxable Fringe Benefit
Taxable Allowance (Maximum allowed is $37 for a phone, $47 for a phone with a business required texting plan, and $77 for a Blackberry)
