Ohio Homeland Security DMA FormPDF DocumentAll applicants that you submit for background check must complete this Declaration of Material Assistance (DMA) Form while looking at the Terrorist Exclusion List (TEL).
Police Record DisclosurePDF DocumentAll Regular Employee applicants must complete this form and submit with 3 other forms needed to start a background check.
SSA-1945 Soc Sec Benefit Awareness StatementLinkAs of Jan 1st, 2005 applicants must complete this form and submit to SAHR with Police Record Disclosure Form, signed employment application, and Ohio Homeland Security DMA Form to start a background check.

Sick Leave Conversion Sign UpWord DocumentCWA Service, Skilled Trades and Maintenance bargaining unit employees are eligible to participate in the CWA Sick Leave Conversion Program. Program allows conversion of sick leave to vacation provided they meet specific requirements.
